Dear Patient:
We appreciate your selection of our office for your complete eye care.
First visits take approximately one hour. Dilation of your eyes will probably be required for your examination. Since the effects of this procedure can take one to two hours to wear off, it may be best to arrange a driver for your return home. Also, bring any current eyeglasses, contacts or eye drops with you at the time of your visit.
We will need each patient to complete a Patient Registration form, a Medical History form and a Current Medication form. These forms are available on this website.
If we will be filing insurance, please bring your current insurance card(s). Government regulations also require that we obtain a copy of your photo ID.
Also, if you require a refraction*, the charge will be $20.00. Please be prepared to pay for the refraction charge as well as any co-pay required by your insurance carrier.

A refraction is the process of determining if there is a need for corrective eyeglasses or contact lenses. It is an essential part of an eye examination and necessary to write a prescription for glasses or contact lenses.
Most medical insurance plans do not cover routine fractions as they consider it a vision charge versus a medical charge. Our office fee for refraction is $20.00 and this fee is collected at the time of service. Should your insurance plan pay for the refraction, you will be refunded accordingly.
